Fill out the pre-workshop evaluation questionnaire. Each participant should do this.

Create an account on the Duke Math Department server
Only one account per team!
Notes: (a) The web server being used for this account page uses an SSL certificate that will not be registered. So you will have to ACCEPT the certificate before you can view the page.
(b) After you submit the registration form, you will receive a username on the confirmation page. Andrew (the systems administrator) will notify you at your contact address when the account has been activated.

Note: You will have to have completed the account registration and received your Duke Math Account name (Step 3) before you can sign up. You can only sign up for one time slot. Signing up for a second will cancel the first. (We will send out information tomorrow on the way we will handle the conference sessions.)
